According to America's ABC News, operator Tanya Graddy died after a detonation inside a furnace that sparked a fire.

An investigation is underway.

The fire was reportedly contained in a small area by the crew, with the refinery kept running.

Graddy's role was reportedly to manage the flow of crude through the plant and the furnaces are part of the first-step "cracking area" of the refinery where crude oil is broken down into gasoline or coke.

The last fatality at the refinery was in 2007 when one of the crude processing units caught fire.

Meanwhile, reports on Friday indicated that the fire near Milford at the Chevron-operated West Texas liquefied petroleum gas pipeline was still burning, a day after a construction crew caused an explosion by accidentally drilling into the pipe.
