Petrobras platform disaster findings

OPERATIONAL and equipment installation failures were blamed in Petrobras' report into the gas explosion on the BW Offshore-operated Cidade de Sao Mateus floating oil production, storage and offloading vessel that killed nine people on February 11.

The bodies of the last three victims of the disaster, which also injured 26 other workers, were recovered by early March according to BW with two crew still receiving medical attention at that time.

Petrobras' resulting report, announced on Friday, found that failures in fluid pumping procedures, along with the incorrect installation of a pipe racket were the main causes of the blast.

The report also covered flaws in safety procedures as some workers evacuated to the platform's pump house after an alarm was triggered.

The disaster was the worst Brazilian oil and gas accident in 14 years.
